Photo Permissions For The 2024- 2025 School Year

Please complete the photo permissions form for the 2024-2025 school year.

When completing the form, we ask that you consider permitting the following consents:

Classroom Consent

Allows staff to take pictures of students for in-class events (i.e., students schedules, classwork, etc.).

School-Based Event Consent

Allows staff to share my student's pictures and videos for school-based events (i.e., videos, photo collages, art/ music/ theater shows).

School-Based Sharing Consent

Allows staff to share my student's pictures and videos from school-based events (i.e., videos, photo collages, art/ music/ theater shows) with other families in the school.

Website/ Social Media Consent

Allows staff to share my students' pictures and videos on our website and on our social media.
